Immunization Requirements

  • Students will not be allowed to attend school without having all required immunizations.                           
     2015 UPDATE:  The new immunization regulations require secondary students (6th- 12th grade) to have Meningococcal (MCV4).  P3, P4, Kindergarten and 1st grade students must have Hepatitis A vaccines.                                                                                                                                                                

    What are the LRSD's immunization requirements?

    When you register your child for school, it will be necessary to provide a copy of the child's complete immunization record to the school.  It is important that any needed immunizations be obtained prior to the day of enrollment. Parents may apply for an exemption in some cases. If you have any questions, please consult the school nurse at any LRSD school.  

    Three and four-year-olds entering the Early Childhood Program must have received all of their vaccines, be in the process for receiving needed doses or show proof that they have applied for an exemption in order to attend school. The following vaccines are required for Early Childhood Program students:

    • Diphtheria
    • Haemophiles influenza type B
    • Hepatitis B
    • Pertussis
    • Poliomyelitis
    • Rubeloa (red measles)
    • Rubella
    • Tetanus
    • Varicella (chickenpox)

    All kindergarten through 12th grade students must show proof of immunization, be in the process for receiving needed doses or show proof of exemption. A certificate by a licensed physician or a public health department acknowledging the immunization must be provided. Required vaccinations include:

    • Dipththeria
    • Hepatitis B
    • Mumps
    • Pertussis
    • Poliomyelitis
    • Rubeloa (red measles)
    • Rubella
    • Tetanus
    • Varicella (chickenpox)


    Are physical exams necessary for my child to enter school?

    All students who are entering a LRSD school for the first time are required to have a physical examination by their doctor or primary health care provider.  Students transferring from another district must provide a copy of a physical exam in their transfer file or obtain a new physical.

    Parents are encouraged to schedule your child's Pre-Kindergarten physical exam early to ensure they are seen by the physician before school begins. You must also request a copy of the report to provide to the school. If you have any questions about this requirement, consult with the school nurse at any of our schools.


    What about Individual Healthcare Plans and Action Plans?

    ndividual Healthcare Plans or Action Plans are required for all students with ongoing health needs (asthma, diabetes, food allergies, seizures, etc.).  Forms are available at the school office and under the Admin section of the LRSD Website. IHP Forms must be signed by your child's physician, school nurse and principal before the student's first day of school. LRSD also accepts any physician's forms, so you do not have to use the LRSD forms to meet state laws.
