Immunization Requirements

When you register your child for school, it will be necessary to provide a copy of the child's complete immunization record to the school.  It is important that any needed immunizations be obtained prior to the day of enrollment. Parents may apply for an exemption in some cases. If you have any questions, please consult the school nurse at any LRSD school.  Students will not be allowed to attend school without having all required immunizations.            

Three and four-year-olds entering the Early Childhood Program must have received all of their vaccines, be in the process for receiving needed doses or show proof that they have applied for an exemption in order to attend school. The following vaccines are required for Early Childhood Program students:

  • Diphtheria

  • Haemophiles influenza type B

  • Hepatits A

  • Hepatitis B

  • Pertussis

  • Poliomyelitis

  • Rubeloa (red measles)

  • Rubella

  • Tetanus

  • Varicella (chickenpox)

All kindergarten through 12th grade students must show proof of immunization, be in the process for receiving needed doses or show proof of exemption. A certificate by a licensed physician or a public health department acknowledging the immunization must be provided. Required vaccinations include:

  • Dipththeria

  • Hepatitis A

  • Hepatitis B

  • Mumps

  • Pertussis

  • Poliomyelitis

  • Rubeloa (red measles)

  • Rubella

  • Tetanus

  • Varicella (chickenpox)

  • Meningococcal (MCV4) (6th-12th grade students)
